Overview:

Specialty Pharmacist

The Specialty Pharmacist provides comprehensive clinical pharmacy services to patients receiving complex and high-cost specialty medications. This role works collaboratively with providers and the interdisciplinary care team to optimize medication therapy, improve clinical outcomes, support medication access and adherence, and ensure safe and cost-effective use of specialty therapies.

 

Key Responsibilities:

• Provide comprehensive medication management for patients receiving specialty medications across a variety of complex disease states.
• Assess appropriateness of specialty medication therapy, including indication, dosing, drug interactions, monitoring, safety, and clinical response.
• Provide ongoing patient monitoring and follow-up to optimize therapy and identify adverse effects, adherence barriers, and opportunities for intervention.
• Collaborate with physicians and other healthcare providers regarding medication selection, therapy optimization, and management of medication-related problems.
• Provide patient education and counseling regarding specialty medications, administration, expected outcomes, adverse effects, monitoring, and adherence.
• Support medication access by identifying and resolving prior authorization, financial assistance, copay, manufacturer assistance, and other barriers to therapy.
• Coordinate with providers, payers, specialty pharmacies, manufacturers, and other stakeholders to facilitate timely initiation and continuation of therapy.
• Promote appropriate utilization and financial stewardship of high-cost and specialty medications.
• Monitor laboratory results, clinical outcomes, and other patient-specific parameters to evaluate medication effectiveness and safety.
• Support specialty pharmacy accreditation, quality improvement, and regulatory requirements.
• Develop and implement clinical protocols, treatment pathways, and standardized specialty medication management processes.

Qualifications: Minimum Education and Experience Requirements::Graduate of an ACPE accredited School of Pharmacy. PharmD or equivalent education and hospital or ambulatory training / experience required, based on practice area of the position. PGY2 residency or equivalent experience required. PGY 2 residency training is preferred. Two to four years-experience in a relevant practice area will be considered equivalent to each year of residency training. Board certification is preferred. Strong written and verbal communication skills. Ability to interact effectively with medical, nursing, and pharmacy staff, pharmacy residents, and students is required. Knowledge and understanding of general hospital and outpatient clinic operations. Ability to integrate clinical services, and educational activities is required.Motor Vehicle Operator Designation::Employees in this position:Will operate vehicles for an assigned business purpose as a non-frequent driverLicensure/Certification/Registration::Registered pharmacist in Florida or eligible for same.Additional Requirement::As a condition of employment within the Pharmacy Services Department of Shands Hospital all employees must be accessible by telephone. This requires each employee to provide a valid telephone number to the department Staff Assistant, in order for the department to reach them in the event of hospital need and/or emergency.
