Full job description
Nexus at Belleville is seeking a Per Diem Speech Language Pathologist (SLP) to provide skilled therapy services in a skilled nursing setting. This role offers flexible scheduling based on facility needs.
Responsibilities:
- Evaluate and treat residents with speech, language, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans
- Provide dysphagia assessment and treatment, including diet recommendations
- Document evaluations, progress notes, and treatment plans accurately and timely
- Collaborate with an interdisciplinary care team including nursing and rehabilitation staff
- Educate patients, families, and caregivers on communication and swallowing strategies
- Participate in care planning and discharge planning as needed
Qualifications:
- Master’s degree in Speech-Language Pathology
- Active Illinois SLP license required
- CCC-SLP preferred
- Experience in skilled nursing or long-term care preferred
- Knowledge of dysphagia treatment and rehabilitation techniques
Schedule:
- Per diem / as-needed
- Flexible shifts based on facility census
- May include weekends or holidays
