Job Summary

Nexus at Forest Edge is seeking a compassionate and motivated Speech Language Pathologist (SLP) to join our in-house rehabilitation team in Chicago, Illinois. The ideal candidate will evaluate and treat residents with speech, language, cognitive, communication, and swallowing disorders in a subacute rehabilitation and skilled nursing setting.

This position offers the opportunity to work collaboratively with an interdisciplinary team dedicated to helping patients regain independence and improve quality of life.

Responsibilities

  • Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing disorders
  • Provide skilled speech therapy services in accordance with physician orders and facility policies
  • Perform clinical swallowing evaluations and dysphagia treatment
  • Maintain accurate and timely documentation in compliance with state, federal, and facility guidelines
  • Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and families regarding patient progress and discharge planning
  • Educate patients, caregivers, and staff on communication and swallowing strategies
  • Participate in care plan meetings and interdisciplinary team discussions
  • Ensure compliance with all regulatory and ethical standards

Qualifications

  • Master’s Degree in Speech Language Pathology
  • Current Illinois SLP license required
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Experience in skilled nursing, subacute rehabilitation, or geriatric settings preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
