Job Summary
Nexus at Columbia is seeking a compassionate and motivated Per Diem (PRN) Speech Language Pathologist (SLP) to join our in-house rehabilitation team in Columbia, Illinois. The ideal candidate will evaluate and treat residents with speech, language, cognitive, communication, and swallowing disorders in a subacute rehabilitation and skilled nursing setting.
This per diem role offers flexibility while supporting an interdisciplinary team dedicated to improving patient outcomes and quality of life.
Responsibilities
- Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing disorders
- Provide skilled speech therapy services in accordance with physician orders and facility policies
- Perform clinical swallowing evaluations and dysphagia treatment
- Maintain accurate and timely documentation in compliance with state, federal, and facility guidelines
- Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and families regarding patient progress and discharge planning
- Educate patients, caregivers, and staff on communication and swallowing strategies
- Participate in care plan meetings and interdisciplinary team discussions as needed
- Ensure compliance with all regulatory and ethical standards
Qualifications
- Master’s Degree in Speech Language Pathology
- Current Illinois SLP license required
- Certificate of Clinical Competence (CCC-SLP) preferred
- Prior experience in skilled nursing, subacute rehabilitation, or geriatric settings preferred
- Strong clinical, communication, and interpersonal skills
- Ability to work independently with minimal supervision
