Best Home Health Providers is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to provide high-quality home health services to patients in their homes. The ideal candidate is passionate about helping patients improve communication, swallowing, and overall functional abilities while collaborating with an interdisciplinary healthcare team.
Position Summary
The Speech-Language Pathologist is responsible for evaluating, planning, and implementing speech-language pathology services in accordance with physician orders, agency policies, and professional standards of practice.
Key Responsibilities
Conduct comprehensive speech-language evaluations and assessments.
Develop individualized treatment plans to address speech, language, cognitive, communication, and swallowing disorders.
Implement therapeutic interventions using appropriate techniques and equipment.
Recommend communication devices and adaptive aids when necessary.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated patient care.
Participate in case conferences, in-service trainings, and team meetings as required.
Educate patients, families, caregivers, and healthcare staff regarding treatment plans and home programs.
Complete and submit clinical documentation in a timely manner according to agency standards.
Maintain clinical competency and stay current with best practices in speech-language pathology.
Identify patient and caregiver needs and make referrals to appropriate services when needed.
Supervise Home Health Aides as applicable to the patient's speech therapy plan of care.
Perform other related duties as assigned.
Requirements
Master's or Doctoral Degree in Speech-Language Pathology.
Current Speech-Language Pathologist license in the state where services are provided.
Minimum of one (1) year of clinical experience in Speech-Language Pathology preferred.
Strong verbal and written communication skills.
Excellent organizational and time management skills.
Current CPR certification.
Valid driver's license, reliable transportation, and current auto insurance.
