Join a growing home-based rehabilitation team dedicated to helping adults maintain and improve their communication, cognitive, and swallowing abilities in the comfort of their own homes.
We are seeking a licensed Speech-Language Pathologist to provide individualized therapy services to patients throughout the Jamaica, Queens area. This position offers flexibility, independence, and the opportunity to deliver meaningful care in patients' everyday environments.
Key Responsibilities:
Patient Assessment:
Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations.
Analyze findings to develop individualized treatment recommendations.
Identify functional barriers impacting communication and daily activities.
Therapy Services:
Provide evidence-based interventions tailored to each patient's goals.
Address speech, language, voice, cognitive, and communication challenges.
Monitor patient progress and modify treatment plans as appropriate.
Swallowing and Feeding Support:
Assess swallowing function and identify safety concerns.
Provide recommendations regarding diet consistency and compensatory strategies.
Educate patients and family members on safe swallowing practices.
Care Coordination:
Communicate with physicians, caregivers, and interdisciplinary team members as needed.
Participate in ongoing care planning to support positive patient outcomes.
Ensure continuity of care through timely follow-up and reassessment.
Clinical Documentation:
Maintain accurate and compliant clinical records.
Complete visit notes and required documentation within established timelines.
Adhere to applicable professional, agency, and regulatory standards.

Qualifications:
Required:
Current New York State Speech-Language Pathologist license.
