Park Bend Rehabilitation and Healthcare Center is seeking a Per Diem Speech-Language Pathologist (SLP) to join our in-house rehabilitation team in a subacute/skilled nursing setting.

The SLP will provide skilled speech, language, cognitive-communication, and swallowing services to residents with a variety of medical and rehabilitation needs. The ideal candidate is compassionate, clinically skilled, dependable, and committed to providing high-quality, patient-centered care.

Responsibilities

• Complete comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop and implement individualized treatment plans.
• Provide skilled speech-language and dysphagia therapy.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate residents, families, and caregivers regarding treatment and safety recommendations.
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team.
• Participate in care planning and discharge planning as appropriate.
• Complete accurate and timely clinical documentation.
• Maintain compliance with applicable professional and regulatory standards.
• Promote resident safety, dignity, independence, and quality of life.

Qualifications

• Current Texas Speech-Language Pathologist license.
• Master's degree in Speech-Language Pathology from an accredited program.
• ASHA certification/eligibility as applicable.
• Previous SNF, subacute, rehabilitation, or healthcare experience preferred.
• Strong communication, organization, and clinical skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
