Seeking a PRN Speech Language Pathologist (SLP) to join the in-houserehab team at North Bay Post Acute, a dynamic skilled Nursing Facility in Petaluma, CA! Ideally needing 10-20 hours/week consistently. All availability welcomed!
SLP Responsibilities:
- Evaluate, diagnose, and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders
- Develop and implement individualized treatment plans based on patient needs and physician orders
- Provide skilled therapy services to improve communication, cognition, and safe swallowing function
- Perform clinical swallow evaluations and recommend appropriate diet modifications and compensatory strategies
- Collaborate with interdisciplinary team members (PT, OT, nursing, physicians) to ensure coordinated care
- Document evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards
- Educate patients, families, and caregivers on treatment plans, safety strategies, and carryover techniques
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes
- Ensure compliance with all federal, state, and facility guidelines (e.g., Medicare, Medicaid, HIPAA)
- Participate in care planning meetings and contribute to discharge planning
- Maintain productivity standards and meet documentation timelines
