Job Overview

Tattnall Health Center is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Speech-Language Pathologists to provide high-quality therapy services in our skilled nursing and subacute rehabilitation setting.

As a Speech-Language Pathologist, you will evaluate, diagnose, and treat residents with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. You will collaborate with an interdisciplinary team to develop individualized treatment plans that promote functional recovery, safety, and improved quality of life following illness, injury, surgery, or hospitalization.

Essential Duties and Responsibilities

  • Perform comprehensive evaluations of speech, language, cognitive-communication, and swallowing function

  • Develop and implement individualized treatment plans based on physician orders, clinical findings, and resident goals

  • Provide evidence-based interventions for speech, language, cognition, voice, fluency, and dysphagia disorders

  • Assess swallowing function and recommend appropriate diet textures, strategies, and safety precautions

  • Monitor resident progress and modify treatment plans to maximize functional outcomes

  • Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines

  • Collaborate with physicians, nursing staff, physical therapists, occupational therapists, dietitians, and other interdisciplinary team members to coordinate resident care

  • Educate residents, families, and caregivers on communication strategies, swallowing safety, cognitive rehabilitation techniques, and home exercise programs

  • Participate in interdisciplinary care conferences, family meetings, and discharge planning

  • Maintain compliance with all professional, regulatory, and facility policies and standards

  • Support a resident-centered rehabilitation environment focused on quality outcomes and clinical excellence

Qualifications

Required

  • Master's degree in Speech-Language Pathology from an accredited program

  • Current Georgia Speech-Language Pathologist license in good standing

  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; Clinical Fellowship (CF) candidates may be considered

  • Current CPR certification or willingness to obtain upon hire

  • Strong clinical assessment, communication, and documentation skills

  • Ability to work effectively within an interdisciplinary healthcare team

Preferred

  • Experience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation

  • Experience with dysphagia management and cognitive-communication disorders

  • Familiarity with Medicare documentation and reimbursement guidelines

  • Experience working with geriatric and medically complex populations
