Position Summary
Tattnall Health Center is seeking a compassionate and motivated Speech Language Pathologist (SLP) to join our rehabilitation team in both Part Time and Per Diem capacities. The Speech Language Pathologist will evaluate residents, develop individualized treatment plans, and provide skilled therapeutic interventions designed to improve communication, cognition, swallowing function, and overall quality of life.

Responsibilities
- Evaluate residents and develop individualized speech therapy treatment plans
- Assess and treat speech, language, cognitive, communication, and swallowing disorders
- Establish functional goals and implement appropriate therapeutic interventions
- Provide skilled speech therapy services to improve communication abilities, cognitive functioning, and safe swallowing techniques
- Assess resident progress and modify treatment plans as appropriate
- Complete evaluations, progress notes, recertifications, and discharge summaries accurately and timely
- Educate residents, families, and caregivers on communication strategies, cognitive support techniques, and swallowing safety recommendations
- Collaborate with physicians, nursing staff, therapists, dietitians, and interdisciplinary team members to support resident care plans
- Ensure compliance with all federal, state, and facility regulations and standards
Qualifications
- Current Georgia Speech Language Pathologist license in good standing
- Master’s degree from an accredited Speech Language Pathology program
- Certificate of Clinical Competence (CCC-SLP) preferred
- Previous experience in skilled nursing, rehabilitation, or long-term care preferred
- Strong communication and interpersonal skills
- Ability to work independently and within a multidisciplinary team
- CPR certification preferred
