    Job Description

    The Speech-Language Pathologist is responsible for assessing, diagnosing, and treating speech, language, voice, cognitive-communication, and swallowing disorders in residents of a skilled nursing facility. The SLP collaborates with an interdisciplinary team to develop individualized treatment plans that promote functional independence and safe participation in daily activities.
     
    Key Responsibilities
    • Perform comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing function.
    • Develop individualized treatment plans based on evaluation findings and physician orders.
    • Provide evidence-based therapy for:
    • Dysphagia (swallowing disorders)
    • Aphasia
    • Dysarthria
    • Apraxia of speech
    • Cognitive-communication deficits
    • Voice disorders
    • Conduct bedside swallowing evaluations and recommend appropriate diet textures and liquid consistencies.
    • Educate residents, families, and caregivers regarding communication and swallowing strategies.
    • Document evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in accordance with facility, payer, and regulatory requirements.
    • Participate in interdisciplinary care plan meetings and communicate resident progress with physicians, nursing staff, occupational therapists, physical therapists, dietitians, and social workers.
    • Ensure compliance with applicable regulations and standards, including those of Centers for Medicare & Medicaid Services and state licensing boards.
    • Recommend adaptive equipment and compensatory strategies to maximize resident safety and independence.
    • Maintain productivity standards while delivering high-quality patient-centered care.
     
    Qualifications
    • Master's degree in Speech-Language Pathology.
    • Current state licensure as a Speech-Language Pathologist.
    • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association preferred
    • Current CPR certification
    • Experience in geriatrics, rehabilitation, or skilled nursing preferred.
