Concord Village Rehabilitation and Wellness is seeking a Full-Time Speech-Language Pathologist (SLP) to join our in-house rehabilitation team in Concord, Ohio. PRN opportunities are also available!

What You’ll Do

• Evaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders
• Develop individualized plans of care based on each resident’s needs and goals
• Provide skilled SLP services to short-term rehabilitation and long-term care residents
• Collaborate with PT, OT, nursing, physicians, dietary, and other members of the interdisciplinary team
• Provide resident, family, and caregiver education
• Complete accurate and timely clinical documentation
• Participate in discharge planning and interdisciplinary care conferences as appropriate

What We’re Looking For

• Current Ohio SLP license in good standing
• Graduate of an accredited Speech-Language Pathology program
• SNF/LTC experience preferred
• Experience with Net Health and PointClickCare (PCC) preferred
• Strong clinical, communication, and documentation skills
• Team-oriented and resident-focused approach
