Job Title: Speech-Language Pathologist (SLP) - Home Health Reports To: Director of Nursing

Job Summary: The Speech-Language Pathologist (SLP) provides skilled speech and language therapy services to adult and geriatric patients in their homes. The SLP evaluates, diagnoses, and treats speech, language, cognitive-communication, voice, and swallowing disorders to promote optimal recovery, independence, and quality of life.

Key Responsibilities:

Perform initial and ongoing speech therapy evaluations in the patients home. Develop and implement individualized care plans based on patient assessment and physician's orders. Provide therapeutic techniques for the rehabilitation of patients with speech, language, voice, cognitive, and/or swallowing disorders. Instruct patients, families, and caregivers in therapy techniques and strategies. Collaborate with interdisciplinary team members (nurses, occupational therapists, physical therapists, etc.). Maintain accurate and timely documentation in accordance with state and federal regulations, payer requirements, and company policies. Educate patients and families about goals and progress. Reassess and adjust care plans as necessary. Adhere to infection control and safety policies during home visits.

Qualifications:

Current, active license as a Speech-Language Pathologist in the state of Florida . Certificate of Clinical Competence (CCC-SLP) preferred Minimum of 1 year experience in a clinical or home health setting (preferred). CPR certification (required)..

Strong communication and interpersonal skills. Ability to work independently in a home health setting. Organizational and time management skills. Cultural sensitivity and the ability to work with diverse patient populations.
