Key Responsibilities
• Perform comprehensive speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on patient needs and clinical goals
• Provide therapy interventions to improve communication and swallowing safety
• Educate patients and caregivers on communication strategies, swallowing precautions, and home exercises
• Collaborate closely with interdisciplinary care team members
• Monitor patient progress and modify treatment plans as needed
• Maintain timely and accurate documentation in EMR (WellSky)

Qualifications
• Active California Speech-Language Pathology license
• Master’s Degree in Speech-Language Pathology from an accredited program
• CCC-SLP preferred
• Home Health or adult rehabilitation experience preferred, not required
• Strong communication, documentation, and patient education skills
• Valid California driver’s license and reliable transportation
