Centers Home Therapy (CHT) is expanding its footprint and we are seeking Speech Therapist to provide home-based speech therapy services.
Responsibilities:
Make home visits to clients.
Evaluates patients referred for home care services to determine appropriateness for speech therapy services.
Establishes a home care plan including assessment of the appropriateness of the requested services.
Provides rehabilitative services for speech and language disorders to meet established treatment goals.
Evaluate patients’ conditions for swallowing or cognitive deficits.
Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.
Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers.
Keep accurate records of patient’s progress, goals and attendance.
Selects and use appropriate diagnostic and therapeutic techniques and materials.
Documents type of treatment and patient’s response on designated clinical progress notes.
