Interim HealthCare is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our Home Health team serving the Portsmouth, NH area. In this role, you'll deliver one-on-one therapy in patients' homes, helping individuals regain communication, cognitive, and swallowing abilities while maintaining the independence and dignity that come from recovering in a familiar environment.

What You'll Do

As a Home Health Speech-Language Pathologist, you'll help patients maximize their communication, cognition, and swallowing function through individualized therapy plans designed around their personal goals.

Your responsibilities will include:

  • Evaluating patients with speech, language, cognitive, voice, and swallowing disorders.
  • Developing individualized treatment plans that support functional independence and improved quality of life.
  • Providing evidence-based therapy in the comfort of patients' homes.
  • Monitoring patient progress and modifying treatment plans as appropriate.
  • Educating patients and caregivers on therapy techniques, home exercises, and strategies for continued success.
  • Collaborating with physicians, nurses, physical therapists, occupational therapists, and other healthcare professionals to coordinate exceptional patient care.
  • Completing accurate and timely clinical documentation.

Qualifications

  • Master's or Doctoral degree in Speech-Language Pathology
  • Current New Hampshire Speech-Language Pathologist (SLP) license
  • Minimum of one (1) year of speech therapy experience preferred
  • Home health experience is welcomed but not required—we're happy to support therapists transitioning into home health
  • Current CPR Certification
  • Strong clinical assessment, communication, and interpersonal skills
  • Compassionate, patient-centered approach with excellent organizational skills
  • Knowledge of state and federal home health regulations preferred
