Responsibilities
As a Speech-Language Pathologist (SLP), you will:
• Evaluate residents referred for speech-language pathology services
• Assess speech, language, communication, cognitive, and swallowing disorders
• Develop individualized treatment plans based on each resident's needs, abilities, and rehabilitation goals
• Provide direct therapy to improve communication, cognition, and swallowing function
• Establish measurable and clinically appropriate treatment goals
• Monitor resident progress and modify treatment plans as needed
• Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time
• Educate residents, families, and caregivers on treatment plans and therapeutic techniques
• Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary team
• Maintain accurate and complete clinical documentation
• Follow all applicable federal, state, and facility regulations, including HIPAA and resident rights requirements
• Perform other duties as assigned by the Director of Rehabilitation
Qualifications
• Master's degree in Speech-Language Pathology from an accredited college or university
• Current state licensure as a Speech-Language Pathologist
• Certificate of Clinical Competence (CCC-SLP) preferred
• Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred, but not required
• Strong communication, clinical, and interpersonal skills
• Ability to work effectively within an interdisciplinary team
