JOB DESCRIPTION
SPEECH PATHOLOGIST
Responsible for speech therapy services to instruct patients in restoring verbal communication.
QUALIFICATIONS
- A Master’s Degree in Speech Pathology or Audiology from an approved curriculum and certified by the State Department of Education and the American Speech and Hearing Association.
- Must be capable of performing job duties.
KEY RESPONSIBILITIES
- Provides consultation and/or Speech Pathology services on an intermittent basis for home visits.
- Participates in conferences as needed or required to coordinate the care of an individual patient.
- Maintains all records and information relevant to the patient for purposes of the service provided in coordination with the general policies of the agency.
- Completes initial and readmission visit for patients admitted for speech therapy and develops a care plan.
- Completes home visits to evaluate patient’s disability problems.
- Determines treatment goals; develops treatment plan based upon evaluation results and physician’s orders.
- Consults with other health care disciplines concerned with the patient’s care.
- Integrates Speech Pathology treatment into the total patient care plan.
- Administers the Speech Pathology program personally.
- Instructs the patient’s family, or substitute family member concerning those aspects of treatment in which they may work with the patient.
- Conducts ongoing evaluation of the patient’s progress in cooperation with other health team personnel.
12. Report patient’s treatment response to his/her physician and changes treatment plans and goals in accordance with the physician’s orders.
- Records, dates and signs into the patient’s medical record (which is maintained by the agency).
- Evaluation Report.
- Treatment goal and plan.
- Report of each treatment visit and follow up recommendations. If treatment is to be discontinued, the reasons should be included along with follow up recommendations when advisable.
- Submits visit notes within 24 hours of date of visits.
- Submits properly prepared route and time sheets within 24 hours of date of visit.
- Conducts inservices as needed to other agency personnel.
- Attends inservices, seminars, or other meetings as assigned by the immediate Supervisor or other management.
- Maintains confidentiality of information related to business practices, business activities, and personnel.
- Maintains a professional attitude when working with staff, fostering the team philosophy and team environment for all personnel.
- Does not allow personal affairs to interfere with scheduled work time.
- Maintains a professional appearance at all times.
- Provides proper notification and/or advanced notice of absence or tardiness without abuse.
- Participates willingly with special projects and/or overtime work when requested.
- Follows all agency policy and procedures.
