Job Title: Speech-Language Pathologist (SLP)
Job Summary

Evaluates, diagnoses, and treats patients with speech, language, communication, cognitive, voice, and swallowing disorders. Develops and implements individualized treatment plans to improve communication abilities, cognitive function, and swallowing safety while promoting functional independence and quality of life. Collaborates with physicians, rehabilitation staff, nursing personnel, caregivers, and other healthcare professionals to provide comprehensive patient-centered care across a variety of clinical settings.

Requirements
Education
Master's Degree in Speech-Language Pathology from an accredited program.
Licensure/Certification
Current Speech-Language Pathologist (SLP) license in the state of practice.
Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred or required according to facility policy.
Current Basic Life Support (BLS) certification preferred or required by facility policy.
Experience
Previous Speech-Language Pathologist experience preferred.
Experience in skilled nursing, rehabilitation, acute care, inpatient, outpatient, home health, school, pediatric, adult, or geriatric settings preferred.
Experience evaluating and treating speech, language, cognitive, communication, and swallowing disorders preferred.
Experience with electronic medical record (EMR) systems preferred.
Essential Functions
Evaluate patients with speech, language, cognitive, communication, voice, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs and established goals.
Provide evidence-based speech-language pathology interventions to improve communication, cognition, swallowing function, and overall quality of life.
Perform speech, language, cognitive, voice, and swallowing assessments utilizing standardized evaluation tools and clinical judgment.
Conduct modified swallowing evaluations and other swallowing assessments when within scope of practice and facility requirements.
Monitor patient progress and modify treatment plans based on clinical response and rehabilitation goals.
Educate patients, families, and caregivers regarding treatment plans, home exercise programs, communication strategies, and swallowing precautions.
Maintain accurate, timely, and complete clinical documentation in accordance with facility policies, regulatory standards, and professional practice guidelines.
Collaborate with physicians, physical therapists, occupational therapists, nursing staff, case managers, educators, and other healthcare professionals to coordinate patient care.
Participate in interdisciplinary care planning, discharge planning, quality improvement activities, and continuing education.
Maintain patient safety and comply with HIPAA, infection prevention, and facility policies.
May provide therapy services in skilled nursing facilities, rehabilitation centers, hospitals, outpatient clinics, schools, home health, acute care, long-term care, pediatric, adult, or geriatric settings based on facility needs.
