    Position Summary

    We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. In this role, you will evaluate and treat residents with communication, cognitive, and swallowing disorders while collaborating with physicians, nursing staff, families, and other healthcare professionals to improve resident outcomes and quality of life.

    Key Responsibilities

    • Evaluate residents and develop individualized treatment plans
    • Provide speech, language, cognitive, and swallowing therapy services in accordance with physician orders
    • Monitor resident progress and modify treatment plans as appropriate
    • Communicate with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes
    • Complete accurate and timely documentation in compliance with facility, regulatory, and payer requirements
    • Participate in care planning meetings and discharge planning activities
    • Educate residents, families, and caregivers on treatment plans and maintenance programs
    • Maintain compliance with all state practice acts, facility policies, and regulatory requirements
    • Promote resident safety, independence, and quality of life

    Qualifications

    Education & Experience

    • Master's or Doctoral degree in Speech-Language Pathology required

    • Must meet one of the following requirements:

      • Licensed as a Speech-Language Pathologist in the state in which services are provided

    • Experience in skilled nursing, rehabilitation, or long-term care preferred

    • Experience with PointClickCare (PCC) and Casamba preferred

    • Strong clinical, documentation, and communication skills

    Licenses & Certifications

    • Current Speech-Language Pathologist license in good standing
    • Must maintain all required continuing education requirements
    • Must remain in good standing with the applicable State Board
