Job Summary
As a home health Speech Therapist, you will work with patients in the comfort of their own homes, providing personalized care to help them recover from injury or illness. You will provide skilled therapy services to improve quality of life and promote independence in the home setting.
Job Responsibilities

Assess the speech, cognitive and swallowing needs of our home-bound clients with comprehensive evaluation and treatment plan
Promote speech, communicative and swallowing abilities through therapeutic intervention and modify treatment plan as needed
Educate patients, families, caregivers regarding home exercise program, safety
Communicate effectively with all team members
Maintain patient records in accordance with federal and state regulations and company policy to ensure quality standard of care.

Education
· Master’s Degree Speech-Language Pathology
Required Certification
· California Speech-Language Pathology license
· Certificate of Clinical Competence Speech-Language Pathology (CCC-SLP)
· Current driver’s license
· Automobile insurance
· Current CPR/BLS-Healthcare Provider
· Current Physical/TB
