Position – Speech Therapist Location: Holton Manor Elkhorn, WI (on-site position) Status: Full Time, Part Time, Schedule: _________________________________________________________ Job Summary: Under the direction of the Therapy Manager, the Speech Therapist is responsible for skilled therapy services for our growing Inpatient, Outpatient and Home Health Programs, and will be supported by our team of Occupational, Physical and Speech therapists. This position interacts with patients/residents, family members, responsible party and other health team members, while maintaining standards of professional nursing. _________________________________________________________ Compensation: $37.00 - $44.00 per hour, dependent on experience Benefits: Health insurance options designed for flexibility Dental and Vision insurance Company-paid life insurance 401(k) with employer match PTO, True Flexible Scheduling, and Shift Differentials Tuition reimbursement and continuing education Employee Assistance Program (EAP) Referral bonuses, employee discounts, early wage access *Benefits vary by employment status (Full-time, Part-time, PRN)* _________________________________________________________ Key Responsibilities: - Must uphold the professional, legal and ethical standards of practice for speech and language pathology as defined by the ASHA and State Practice Act - Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the rehab department - Performs initial patient examination, evaluation, interpretation of referrals, and creates the initial plan of care for each patient and revises as appropriate - Creates and maintains a patient record for each patient treats - Provides on-site assessment and re-evaluation of each patient’s treatment according to the State or Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate - Coordinates discharge plan decisions - Demonstrates knowledge and appropriate application of precautions and contraindications to speech and language pathology treatment procedures and modalities - As necessary or applicable, supervision of CFYs temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards. - Meets patient care hours (PCH) requirement for your rehab gym, or as directed by Program Manager. - Attends and contributes to patient care, staffing conferences and other related meetings. - Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient. - Consults with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient. - Adapts program and/or plan of care according to the needs of the individual patient. - Promotes a safe environment and enforces the elimination of fire and safety hazards. - Orders supplies and equipment as necessary upon approval by the Program Manager. - Cleans, maintains, identifies, and reports the need for repair of equipment, as necessary. - Puts Patient Service first, ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals needs and rights. - Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. - Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the speech language pathology treatment plan, and completes all related documentation and record keeping regarding these services. - Services include but are not limited to application of therapeutic techniques in areas such as: cognitive-communication, speech intelligibility, oral motor skills, swallowing disorders/dysphagia, perceptual abilities, orientation, memory, pragmatics, psychosocial expression, functional communication. - Coordinates the acquisition of adaptive communication devices and instructs patients in their use. - Facilitates identification of hearing deficits and appropriately refers for assessment, diagnosis rehabilitation and/or personal amplification. - Selects appropriate activities suited to the individuals physical and mental capacity, intelligence level and interest. - Instructs patient in the skills and techniques required for participation in therapeutic activities, and evaluates patient progress, attitudes and behavior as related to rehabilitative potential. - Performs routine reexamination as needed/required to modify/progress plan of treatment. _________________________________________________________ Qualifications: Required: Candidate must possess a Master's degree in Speech-Language Pathology, Communication Disorders, Communicative Disorders or similarly titled area that is consistent and acceptable to the American Speech-Language-Hearing Association. Must possess or be eligible for a Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or be eligible to participate as an ASHA Clinical Fellow. The Speech Language Pathologist must be licensed, certified or credentialed, as required in the state of Wisconsin. Must have excellent communication skills.
