    Description

    JOB SUMMARY

    The Outreach Coordinator conducts outreach to Veterans and community members experiencing or at risk of homelessness, linking them to rapid rehousing, homelessness prevention, and supportive services.

    JOB DUTIES/ESSENTIAL FUNCTIONS

    • Conduct outreach services and street outreach activities.
    • Connect participants with case management, benefits, and housing resources.
    • Verify homelessness status and eligibility.
    • Develop relationships with landlords, shelters, VA offices, GPD providers, CoCs, and community partners.
    • Assist with transportation, communication, self-advocacy, and housing support plans.
    • Attend Stand Down and community outreach events.
    • Collaborate with SSVF staff and perform other assigned duties.

    REQUIRED EDUCATION AND EXPERIENCE

    High School Diploma required. Community Health Worker certification preferred. Experience in outreach, housing, homeless services, or veteran services preferred. Veterans preferred.

    OTHER REQUIREMENTS

    Valid driver license, current vehicle insurance and registration, satisfactory motor vehicle report, and successful criminal history check.
