    Job Title: Registered Nurse (RN) – Start of Care

    Location: Suffolk & Nassau County, NY
    Employment Type: Full-Time, Part-Time, or Per Diem

    Job Description:
    The Start of Care Nurse plays a critical role in initiating home health services for new patients. You will perform comprehensive assessments and develop individualized care plans to ensure continuity and quality of care.

    Always Compassionate Health is a trusted provider of home and community-based healthcare services across New York. We are passionate about delivering high-quality, compassionate care that empowers individuals and strengthens communities. Join our team and help support students in local schools with essential medical care.

    Pay Rates:

    • Initial Start of Care: $150 per visit

    • Follow-Up Visit: $90 per visit

    • 90-Day Post-Operation Visit: $60 per visit

    • Aide Evaluation Visit: $40 per visit

    Responsibilities:

    • Conduct initial patient assessments and complete all required documentation.

    • Develop and implement individualized care plans in compliance with regulatory standards.

    • Educate patients and families on care procedures, medications, and health management.

    • Coordinate with interdisciplinary teams to ensure seamless transitions and ongoing care.

    • Maintain accurate records and ensure compliance with agency and state guidelines.

    Qualifications:

    • Current and valid RN license in New York State.

    • Strong knowledge of OASIS documentation and regulatory requirements.

    • Excellent organizational and communication skills.

    • Ability to travel to patient homes within assigned territory.
