We’re seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. These visits are conversational, consultative, and education-focused— no prescribing, no treatment plans.

What You’ll Do
- Conduct comprehensive in-home wellness assessments
- Educate members on preventive care benefits
- Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations
- Complete documentation using provided EMR tools
Visits are face-to-face and take place where members live—allowing for more personalized, impactful care.
Requirements
- Active State NP license and Board Certification
- Minimum 3 months of wellness exam experience preferred
- Comfort with EMR/documentation systems
- Ability to travel within a 30–50 mile radius
- Bilingual/Spanish Speaking
