Access Services is hiring a full time Street Medicine Outreach Worker who will serve the community throughout Montgomery County, with an office base in Plymouth Meeting, PA. The Street Medicine Outreach Worker will be passionate about serving vulnerable individuals without judgement and will be able to understand how the Social Determinants of Health and the public health system impact people who are currently unhoused. With skills in organization and partnership, this individual will be be able to engage well with people experiencing challenging life circumstances, who may occasionally become escalated. They will possess the grit to remain on the journey with people to improved health and connection to resources. The Street Medicine Outreach Worker willingly serve people living outside, with confidence and awareness that they will encounter a wide range of weather and environmental factors.
Location: Serving in locations across Montgomery County, PA, frequently outside in all weather conditions.
Schedule: Monday-Friday, 40 hours weekly; 8am-4pm
Essential Functions of the Street Medicine Outreach Worker:
- Identify the obstacles/barriers that keep a person from utilizing the healthcare that is available. Decrease those barriers by supporting with transportation to appointments and clinics, and facilitate effective screening, assessment, and connection to resources.
- Partner with unhoused individuals to assess their needs in the areas of physical health and behavioral health, including substance use disorders, including the use of screen and assessment tools.
- Provide individuals with education regarding medical diagnoses, including SUD and Mental Health related diagnoses to improve health literacy and education on resources to support these diagnoses.
- Facilitate connection to the necessary testing and evaluations for any healthcare related needs.
- Support Street Medicine Providers in navigating safely into encampments and other locations where unhoused individuals are living and need care.
- Support in site-based training and education of students who are shadowing or pursuing an internship with Access Services.
- Attend county, committee, and consumer meetings; facilitate multisystem meetings and provide support for complex scenarios.
- Attend regular team meetings and case reviews with both the Street Medicine and Street Outreach teams.
- Partner with The Montgomery County Overdose Response team and other Drug and Alcohol providers to navigate treatment for those with Substance Use Disorder and Opioid Use Disorder.
- Education related to HIV, Hepatitis, and TB is to be provided to people served when appropriate, as well as referral to testing and treatment as necessary.
- Complete all case notes, documentation, and outcomes, including statistical reports and billing reports as needed.
Other:
- Maintain regular and predictable attendance, and work scheduled overtime, or be available for unplanned overtime as necessary, to meet program needs.
- This is a client-facing position that does not include remote or hybrid work schedule options. This job requires a full-time, on-site presence due to the nature of the role and responsibilities.
- This position requires the ability to travel during a normal workday to successfully carry out job responsibilities. Therefore, the Street Medicine Outreach Worker will need to drive their personal vehicle and/or an agency-owned vehicle to meet these expectations. This person will be expected to always drive safely and meet eligibility criteria in the agency Motor Vehicle Policy.
Requirements
The selected candidate must meet all qualifying and ongoing job criteria including background checks and a driving history record.
Education:
- Bachelor’s degree in public health, social work, or a related field required; Master’s degree in public health preferred.
Certification:
- American Society of Addiction Medicine (ASAM) training or certification preferred.
Experience:
- Experience working with individuals with housing instability or who struggle with substance use disorder and/or mental health diagnoses is desirable.
Driving Requirements:
- A valid US driver’s license for at least two years, not including time with permit.
- No serious traffic convictions within the past three years. (Serious convictions include, but are not limited to, DUI, reckless driving, leaving the scene of an accident, or three or more traffic violations and/or at-fault accidents within that period).
- Access to a reliable vehicle during work hours. Vehicle must have valid registration and inspection stickers.
- Valid auto insurance. You will be asked to provide proof that you are a covered driver on the policy.
Knowledge, Skills, and Abilities:
- Proficiency in, or ability to learn and use, workplace applications, platforms and technologies.
- Knowledge of Microsoft Office Suite including Word and Outlook, and Evolv.
- Knowledge of homelessness and housing system and the ability to enhance understanding to maximize the level of service delivery.
- Skilled in person centered screening and assessment.
- Ability to work as part of a team.
Essential Working Conditions/Physical Demands:
- Work in program sites and the community routinely providing direct care and/or support with individuals.
- Stand, sit or run by self, or provide proper care and supervision with an individual.
- Able to move and stay with an individual to keep them safe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations.
- Able to assist an individual who has fallen, regardless of weight or size.
- Walk, climb stairs by self, or provide direct care and/or support.
- Work in a standard office environment and utilize desktop computer equipment daily.
- Able to lift or move items associated with sedentary and light work (exert up to 20 lbs.)
- Travel to offsite locations and regional office locations if necessary.

- $22/hourly, with rates based on relevant work and educational experience.
