Summary:
Responsible for coordinating a comprehensive, evidence based Joint Commission Disease Specific Certified Stroke program, New York State Department of Health Certified Stroke Center and compliant with American Heart Association/American Stroke Association Gold standards for Stroke Get With the Guidelines. This position coordinates the development of the program including leading the multidisciplinary teams, collaborating with the Department of Neurology, Neurosurgery, and Interventional Radiology for the development and implementation of protocols, policies, order sets, and educational materials. The Stroke Program Coordinator provides community outreach recommendations, performance improvement support, and coordinates quality outcomes tracking and reporting in accordance with Joint Commission, New York State and American Heart Association/American Stroke Association standards. Population based initiatives conducted depending on scope of services in the department. In conjunction, the position is also a Joint Replacement Program Coordinator, promoting comprehensive evidence-based care for all aspects of the continuum of care beginning before surgery through one-year post-op, including Pre-admission testing, peri-op services, inpatient units, post-acute care and all other applicable entities in accordance with CMS guidelines.
Responsibilities:
2. Provides leadership and education for the stroke multidisciplinary team to ensure compliance with stroke protocols, evidence-based standards, and regulatory agency requirements.
3. Develops and maintains a stroke database for Joint Commission/ CMS core measures, meaningful use performance measures, New York State Department of Health stroke center designation and American Heart Association / American Stroke Association Get With the Guidelines for Stroke. Submits stroke process and outcome performance measures to Joint Commission/ CMS, New York State Department of Health, American Heart Association / American Stroke Association
4. Monitors stroke care from emergency department admission through treatment and stabilization to discharge or transfer to a higher level of care.
5. Applies clinical knowledge to accurately collect a wide variety of stroke clinical performance process and outcome measures from medical records within the designated time frames.
6. Provides education and collaborates with medical and hospital staff including Department of Neurology and Neurosurgery to coordinate the implementation of practice changes as needed.
7.Prepares and participates in the tri-annual Quality performance improvement council report from the Department of Neurology and Neurosurgery.
8. Collaborates with the multidisciplinary stroke care team and Department of Neurology and Neurosurgery to develop protocols, policies, and order sets to support evidence-based stroke care.
9. Participates with system Ortho Coordinators in reporting pre & post operation joint patient outcomes to CMS by supporting patient tracking and enrollment in database platforms.
10. Serves as a liaison between OR leadership, ORs, patients within the service line, and the physicians� offices and care teams.
11. Responsible to identify and escalate concerns as needed regarding patient care and experience, reporting patient experience and service concerns to OR leadership and document occurrences in the electronic health record.
12. Support s total joint patients and their families navigate the many facets of total joint surgery through pre and post operative education utilizing the database systems and or other educational platforms.
13. Coordinates and collaborate transitions between the doctors� office, hospital as well as rehab and home care.
