Job Summary:

The Coor - Stroke Outreach is a registered nurse who is responsible, under the direction of the Neuroscience program Manager and the Comprehensive Stroke Program Medical Director, for the development of and maintenance of partnerships with WellStar system hospitals, outlying hospitals and EMS carriers serving our community and beyond. Implementation of a system wide cerebrovascular management pathway including triage to appropriate level of care will be a main feature of this position. The outreach coordinator will provide immediate feedback on patient outcomes for referring facility practitioners and EMS personnel. Additionally, the outreach coordinator will provide staff education for the implementation of teleneurology protocols for the WellStar Health System. This position utilizes the Stroke Registry in the development of outreach and education programs for the community and professionally within the WellStar Health System. The outreach coordinator will support the collection of data for the Stroke Registry.

Core Responsibilities and Essential Functions:

Professional and Public Education

Participates in regional Stroke education and outreach efforts

Develops educational programs with other hospital team members and staff within the guidelines of WellStar Health System

Works collaboratively with the WellStar Corporate and Community Health division to facilitate public programs and events

Coordinates program initiatives in public education and Stroke prevention efforts

Develops strategic marketing plans in collaboration with WellStar marketing team to enhance communication of services offered at WellStar Kennestone Hospital

Participates in yearly WellStar Neurosymposium, helping to obtain speakers and sponsors.

Works within the Georgia Stroke Professional Alliance Association in efforts to better the care of the stroke patients

Participates in Advanced Stroke Life Support class offered for WellStar employees as well as EMS partners

Participation in EMS week and Stroke month activities

Participates in other professional education courses in stroke and cerebrovascular care that may be conducted or sponsored by the hospital

Collaborates with advanced practice nurses and department specific educators to identify and mitigate education deficits related to the care cerebrovascular patient.

Collaborates with the EMS Coordinator in supporting pre-hospital providers related to the

pre-hospital care of the stroke patient.

Clinical

Attends Stroke Alerts and serve as a resource person to enhance stroke team performance

Serves as a resource to patients and families as needed to ensure optimal Stroke care provision to our patients

Provide immediate feedback on patient outcomes to referring facility practitioners and EMS personnel, adherence to transfer protocols and trouble shoot opportunities for improvement system wide.

Facilitates the stroke alert process at sister hospitals in effort to improve transfer times for appropriate patients

Assist in developing connectivity through telestroke services to outlying hospitals.

Provide feedback to physicians and EMS after transfers have occurred in order to improve communication back to community facilities and physicians.

Data Abstraction

Reviews stroke admission records for data collection and abstraction for performance improvement and appropriateness of transfer. Analyzes records for quality assurance as stipulated by The Joint Commission.

Data abstraction will be used to improve door to needle times in the delivery of IV t-PA at sister hospitals as well as improved efficiency of transferred patients and other cerebrovascular procedures as measured by the Comprehensive Stroke Program performance metrics.

Review transfer process and times on all transferred patients with Patient Flow Director for opportunities to improve and report monthly on success and opportunities.

Performs other duties as assigned

Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:

  • Diploma (Nurse) Nursing or Accredited Program Nursing or Bachelors Nursing

Required Minimum License(s) and Certification(s):

All certifications are required upon hire unless otherwise stated.

  • BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
  • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact

Required Minimum Experience:

Minimum 3 years of clinical nursing in emergency, critical care and/or Neurovascular patient care area is required.

Required Minimum Skills:

Knowledge of Comprehensive Stroke Program elements as well as neurovascular terminology

Excellent interpersonal skills are necessary to interact productively with both internal and external personnel at all levels of the organization, patients, families, physicians and staff.

Organizational skills to necessary to develop educational opportunities for the community and Stroke care providers as well.

Working knowledge of Microsoft Excel, word and power point is required.
