The Case Manager Position will provide a wide range of case management services for individuals with Substance Abuse problems and/or individuals infected with HIV. Serves as an advocate for the client and their family. Will promote available services for the client within the agency and will refer the client for services outside the agency when necessary. Assists Regional Coordinator in developing coalitions and consortiums to enhance the continuum of care for clients.
ESSENTIAL RESPONSIBILITIES:
Facilitates admission (into program) and/or referral (e.g., inpatient treatment, mental health counseling) to appropriate services to assist participants.
Prepares confidential and non-confidential correspondence in a professional style and format.
Inputs data into databases correctly and within designated time frame.
Makes appointments and referrals for clients as needed.
Maintains effective filing system and files necessary information according to Agency policies and procedures
Provides HIV/STD testing in the community
Complies with DSHS standards for charting and case management
Records client sensitive information using approved methods.
Assists supervisor in preparing programmatic reports in an accurate and timely manner.
Documents all activities provided to participants according to Agency standards and procedures
Communicates readily with Supervisor on programmatic, agency interpersonal and significant client information.
Communicates with all community members: such as clients, general public, and other services providers in a friendly, cooperative and professional manner to ensure non-duplication of services.
Prepares monthly reports in a timely manner as requested by supervisor.
Prepares time reports and mileage accurately and on time.
Serves as an advocate for the clients at all times, avoids derogatory and/or inappropriate remarks regarding clients.
Ensures that all services given to clients are based on client eligibility guidelines.
Must be familiar with Performance Measures, objectives and grant requirements.
Must be familiar with all currently approved service caps.
Makes referrals to social service agencies that provide services not obtainable from Special Health Resources.
Ensures that all clients have accurate and up to date consents and forms in charts.
Provides intervention strategies designed to reduce substance use and related negative behaviors to client and family members as appropriate
Involved with or serves on other local consortiums, coalitions and community organizations.
Assists in monitoring the quantitative goals and targeted risk groups to assure compliance with grant requirements.
Provides participant transportation to activities in a safe and appropriate manner.
Conducts community presentations as necessary
