Responsibilities/Job Description:

Fairview has an immediate opening for a Structural Heart Clinic RN at the Fairview Heart Care Clinic located on St. John's Hospital Campus in Maplewood, MN.

 

This is a 1.0 FTE (80 hours per two week pay period), day shift, benefits eligible opening. We believe the schedule will be Monday - Friday 8:00 am to 4:30 pm 

 

The structural heart RN role provides thorough, patient and family centered care for patients with structural heart conditions- specifically valve disease. The valve program coordinator works to maximize patient’s experience and health outcomes from initial referral, through consult, procedure, and follow up care.

This is a specialized RN role requiring knowledge of general cardiology, valvular disease, heart failure, and procedural/surgical treatments.



Responsibilities

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.
  • Actively participates in creating and implementing improvements.
  • Coordinates care across the healthcare continuum within designated sub-specialty population(s). Serves as a single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services.
  • Performs patient and family needs analysis. Reviews medications for accuracy, facilitates medication refills, and updates patient's EMR. Verifies results to ensure all necessary tests are completed and the results are available for the providers.
  • Leads a team approach to pre-visit planning. Provides patient/family education that is shared and documented regarding specific diagnosis, treatments, medication management, plan of care and transitions of care. Develops and engages patient and family in plan of care, involving multidisciplinary team. Completes post visit wrap-up and determines necessary follow up appointments and/or treatments. Works to maintain primary care/referring physician involvement through ongoing communication. Works at highest level of scope of practice and skills.
  • Manages patient care in-between clinic visits including communication with primary and referring providers. Participates in and works to individualized shared plan of care. Collaborates with interdisciplinary team. Anticipates and documents patient needs for the next visit. Verifies results to ensure all necessary tests are completed and the results are available for the providers. Provides protocol based care management. Supports care provided to patient in transition of inpatient and outpatient settings through shared communication of plan of care and patient/family needs. May participate in the inpatient care conferences when indicated.
  • Promotes safe, satisfying patient centered care. Designated single point of contact. Applies standard best practices at all levels of care. Shares plan of care and related information. Works with patients and families to navigate throughout the care continuum.
  • Completes visit closure and summarizes visit. Provides patient/family education per assessment of need. Collaborates with care team to maximize patient outcomes. Ensures medication list is reconciled and provides patient with a hard copy. Ensures appointment follow-up is appropriate and timely.
  • Completes pre-visit planning to optimize patient visits. Ensures that appropriate authorization for procedures/devices and medications have been obtained prior to services needed. Ensures all pertinent patient information/tests at time of appointment. Ensures that patient/family needs are communicated to the provider. Verifies appropriate appointment types are made.
  • - Serve as the central point of contact for patients and families during pre and post procedure care including results review, follow up care, registry requirements, medication management, and procedure scheduling.

    - Evaluates all incoming referrals to valve clinic by assessing severity of valve disease based on clinic documentation and echocardiogram metrics, triage patients as appropriate for timely evaluation of valvular diagnosis.

  • Participate in RN tandem visits with providers for consultation, pre op education and prep, and post-procedure follow up visits for TAVR, TEER, and TTVR.

  • Develop as well as present education materials for staff, patients, and referring providers in a variety of mediums. Coordinate with outside vendors to develop and enhance staff knowledge on specific device/implant use as well as to prepare staff for launch of new therapies.

  •  Coordinate and host provider led education events with referring providers around the metro area.

    - Monitor quality measures of care for patients undergoing structural heart procedures and assists in adjusting practice according to data. Participates in national registry and ensures patients are meeting required visits per registry requirements.

  • Collaborate across multidisciplinary team to provide cohesive coordination and understanding of patients’ care. This includes preparing and facilitating multidisciplinary valve case conference


Required Qualifications

  • Bachelors Degree in Nursing or 3-5 years of ICU experience
  • Proven Experience with an EMR
  • Excellent assessment and care coordination skills. Understanding or willing to learn important criteria (inclusion and exclusion) required for surgical or percutaneous valve therapy.
  • Understanding basic CTA requirements/risks/results and TEE requirements/risks/results prior to either surgical or percutaneous valve therapy.
  • Basic Life Support (American Heart Assoc or Red Cross)
  • MN Registered Nurse (RN) License

Preferred Qualifications

  • 1 year ICU experience
  • Computer Skills: Basic ability to use a computer and applications that are associated with performing basic work tasks (navigate in Windows, Outlook, epic EHR, etc.).
  • Critical Thinking: Gathers and integrates critical information, recognizing and addressing underlying assumptions of others, to arrive at effective solutions; breaks down problems into simple tasks or activities; analyzes relationships among several parts of a situation.
  • Interpersonal Communication: Skilled in developing effective rapport with customers, co- workers, or families, actively listening to develop a positive connection.
  • Nursing Process: Utilizes the nursing process to provide holistic, compassionate, safe, high quality, population based care.
  • Patient Education: Provides patient educational information appropriate to the developmental level, learning needs, readiness, cultural values, and beliefs of patients and families.

 

Qualifications:

$42.41- $59.87 Hourly
