    Description

    The Unit Manager is responsible for the overall clinical and administrative management, ensuring the highest standards of quality care, regulatory compliance, and resident satisfaction. This role oversees nursing staff, coordinates interdisciplinary care, and supports a safe, efficient, and therapeutic environment for residents transitioning from hospital to home or long-term care.

    Key Responsibilities

    • Clinical Leadership
    • Direct and supervise nursing care on the subacute unit to ensure adherence to standards of practice, policies, and procedures.
    • Oversee resident admissions, transfers, and discharges, ensuring thorough assessments and accurate documentation.
    • Collaborate with physicians, therapists, and interdisciplinary team members to develop, implement, and monitor individualized care plans.
    • Monitor resident progress and intervene as necessary to promote recovery and prevent rehospitalization.
    Staff Management
    • Provide leadership, coaching, and mentorship to nursing staff, including LPNs, and CNAs.
    • Ensure adequate staffing, scheduling, and assignment of responsibilities.
    • Conduct staff performance evaluations, provide feedback, and implement corrective actions as needed.
    • Promote professional development and ongoing education for staff.
    Regulatory Compliance & Quality Assurance
    • Ensure compliance with state (NJ DOH) and federal (CMS) regulations, as well as facility policies.
    • Participate in audits, surveys, and quality improvement initiatives (QAPI).
    • Maintain accurate documentation for resident care, incidents, and regulatory reporting.
    • Monitor infection control practices and medication administration for adherence to standards.
    Resident & Family Relations
    • Serve as a primary point of contact for residents and families regarding care needs and concerns.
    • Promote resident rights, dignity, and individualized care.
    • Facilitate care conferences and family meetings to ensure clear communication and involvement in care planning.
    Administrative Duties
    • Track unit metrics, including rehospitalization rates, falls, infections, and other clinical indicators.
    • Report regularly to nursing leadership on unit operations, staffing, and quality outcomes.
    Qualifications
    • Current State of New Jersey RN license in good standing.
    • Minimum 2-3 years (preferred but not necessary) of nursing experience in subacute/rehabilitation or long-term care; at least 1 year in a supervisory/management role preferred.
    • Knowledge of NJ DOH and CMS regulations.
    • Excellent leadership, organizational, and communication skills.
    • Ability to manage multiple priorities and lead in a fast-paced environment.
