Substance Abuse Counselor: Eastwood Behavioral Health
Eastwood staff support each other as we learn and grow personally/professionally. Under the supervision of the Lead clinician, completes Intakes, group therapy, didactic, case mgmt, and individual therapy. Works with treatment team to resolve underlying issues, improve client lives and resources, and develop an aftercare plan that supports each area of their lives. We are grateful to help the most vulnerable clients to improve their lives, and the severity of consequences often means we have the greatest impact.
Eastwood utilizes multiple recovery pathways, MOUD, and contingency management to help each client find their path to recovery.
Responsibilities:
Provides assigned patient counseling services and case management.
Systematically and continually assesses the patient's coping mechanisms and progress.
Utilizes appropriate teaching methods and/or referral sources.
Coordinates patient's discharge planning needs and readiness for discharge with other healthcare members.
Qualifications:
