Responsible for planning and implementing activities for clients with mental illness and co-occurring substance abuse disorders, supporting their efforts to maintain or improve daily living skills, interdependence, community integration, and sobriety.
This position does not have direct staff reports.
Provide substance abuse prevention training incorporating 12-step programs and evidence-based practices for co-occurring disorders.
Transport clients to community activities and AA/NA meetings.
Develop a monthly activity calendar, identifying community resources to support sobriety.
Conduct drug screenings as necessary.
Complete required documentation, including individual treatment plans and progress notes.
Organize and facilitate social and recreational activities appropriate for ACT clientele.
Provide case management for assigned clients, coordinating treatment plans and ensuring real-time adjustments based on evolving client needs.
Conduct comprehensive assessments across all domains of a person’s life.
Offer on-call crisis intervention, covering nighttime and weekend hours when assigned.
Become an expert in the ACT philosophy, maintaining high fidelity to the ACT Model of Care.
Planning and implementing social, recreational, and training activities that support sobriety.
Ensuring completion of ITP goals and required documentation.
Providing substance abuse education.
Coordinating 12-step community activities.
Bachelor’s degree in Psychology, Human Services, or Addiction Counseling preferred.
1-3 years of equivalent work experience may be considered.
Minimum ADT Trainee Status or CAC, CAC-AD certification required.
Two to three months of on-the-job training (OJT) required for efficient job performance.
Certified or eligible for certification in addictions treatment.
Must have a valid driver’s license and a clean driving record.
Ability to work independently with minimal supervision.
Knowledge of mental illness and addictions is essential for success in this role.
