Responsibilities:
The Substance Use and STD Navigator provides direct services to identified patients with substance use disorder or sexually transmitted diseases at Community Medical Center's (CMC) Emergency and inpatient units. Develops a plan of care to coordinate outreach screening, assessment, referral, treatment, education, data collection, and utilization of community based resources to facilitate with compliance of the medical care plan. The position requires collaborative efforts with physicians, Case Management, Social Workers, and other internal and external personnel to promote appropriate utilization of healthcare resources.
Qualifications:
Education
• Associate's Degree in a related field required
• Bachelor's Degree in Social Work, Public Health, or Human Services related field preferred
Experience
• 2 years of substance abuse and/or STD population assessment and referral experience required
• Experience working with community based programs preferred
• Experience in an acute care hospital, public health, or behavioral health clinic preferred
Licenses and Certifications
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
