Valid PPS: Speech Language Pathologist or Clinical or Rehabilitative Services Credential (detailed copy) See below on how to obtain your detailed credential copy. 2. Cover Letter 3. Resume 4. (3) Current Letters of Recommendation 5. TB Test Verification (TB skin test must be no more than 1 yr old and must include administered and date read) OR TB Assessment that is within 60 days of application submission. 6. Fingerprint Clearance Receipt from the OC Dept of Ed "Clearing House". (No other FP Clearance will be accepted. If you do not have FP Clearance thru the OC Dept of Ed, please contact them at (714) 966-4306 for appointment and fees) To obtain a copy of your detailed credential, please go to: 1. www.ctc.ca.gov 2. Click on "Search for an Educator" 3. Enter your "Social Security Number and Birth Date" 4. Select your valid teaching credential for this position and print 5. Upload to your application
