    Intermountain Centers and its statewide affiliates are currently recruiting career-minded individuals interested in opportunities within the largest adult and child service continuum in Arizona.

    General Summary: Under the supervision of the SUD Project Manager, the SUD Justice, Hospital & Community liaison is responsible for outreach to members who are involved in the criminal justice system, hospitals, other community settings or are currently in jail. The Liaison will coordinate with the Pima County and State courts, hospital staff, the member, and the treatment Team as applicable to ensure timely follow through on community re-entry, appointment adherence and additional support during follow up appointments. The liaison will coordinate and monitor successful compliance with mental and physical health recommendations.

    JOB RESPONSIBILITIES:

    • Successfully coordinate with criminal justice resources, hospitals and other applicable community settings.
    • Coordinate outpatient and peer support services to members to avoid gaps in services and to provide continuity of care (including but not limited to: diversion, educational activities, med cueing, environmental assessments, transportation, referrals, ART meetings, etc.).
    • Facilitate psychoeducational groups for members involved in the criminal justice system to decrease recidivism.
    • Complete documentation for behavioral health records in accordance with agency and grant requirements.
    • Participate in the assessment, evaluation, and planning of patient care. Complete SDOH assessment.
    • Provide case management, health promotions, skills training, and assessment when engaging with members involved in identified settings.
    • Maintain a minimum number of documented client contact hours per week.
    • Maintain close collaboration with referral sources of identified settings to continue referrals.
    • Provide member advocacy and education as needed, to ensure delivery of required services.
    • Perform other related duties in accordance with agency growth and changes.

    Perform other duties as assigned or necessary as it relates to the general nature of the position.

    QUALIFICATIONS:

    • Minimum Education Required: Bachelor's degree in a behavioral health related field OR BHT level with two years of behavioral health experience.

    REGULATORY:

    • Minimum 21years of age.
    • Able to pass a minimum 5 panel drug test & submit to regular screening.
    • DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
    • Maintain a clear Central Registry Background Check.
    • CPR, First Aid, (must possess upon hire and maintain throughout employment).
    • Current, valid Arizona Drivers License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements.
