Description

Collaborates with all members of the health care delivery team to enhance the provision of patient care services and safety from admission to discharge. Functions as the lead member and supervisor of the care coordination team to ensure high quality patient care, appropriate length of stay, excellent resource utilization, and ensure reimbursement for services rendered. Adjusts assignment of staff to assure coverage. Serves as a resource to the hospital for utilization management issues. Estimated salary range for this position is $92996.80 - $123685.74 / year depending on experience.

Qualifications

Degrees:

  • Bachelors.

Licenses & Certifications:

  • ACMA ACM Certification.
  • CCMC Case Manager.
  • ACMA Case Management Administrator Certification.
  • Registered Nurse.
  • NAHQ Certified Professional in Healthcare Quality.
  • ANCC Nursing Case Management.
  • ANCC Nurse Executive, Advanced Certification.
  • ANCC Nurse Executive - Board Certified.
  • NACCM Care Manager Certified.

Additional Qualifications:

  • BSN-Bachelor of Science in Nursing required.
  • One of the listed certifications required within 1 year of job entry date.
  • Knowledge of reimbursement principles, discharge planning process, InterQual and/or Milliman criteria.
  • Advanced written, verbal and interpersonal skills.
  • Must be able to deal effectively with coworkers and medical staff.
  • Demonstrates leadership ability, professionalism and integrity.
