    Description

    The Care Manager Supervisor oversees care coordination services for Health Home members,

    ensuring high-quality, compliant, and person-centered care. This role includes supervising care

    managers, conducting audits, maintaining documentation standards, and supporting staff

    development and training.


    Key Responsibilities:

    • Supervise and provide guidance to care managers, offering consultation and ongoing

    support.

    • Ensure all program staff complete required initial and ongoing training.

    • Deliver New Employee Orientation and conduct periodic in-service trainings.

    • Monitor service quality by reviewing care plans, progress notes, case documentation,

    and incident reports.

    • Conduct caseload audits to ensure compliance with standards and continuity of care.

    • Perform spot checks of care manager caseloads to assess member satisfaction and

    service delivery.

    • Review documentation to ensure adherence to regulatory and quality standards.

    • Manage and assign caseloads to ensure appropriate service delivery for enrolled,

    pended, and diligent search members, in accordance with Health Home policies.

    • Oversee the Person-Centered Planning process to ensure member-focused care.

    • Provide direct care coordination services to members as needed.

    • Review and approve assessments, care plans, reassessments, consent forms, and

    encounter notes requiring supervisory sign-off.

    • Process and follow up on incident reports, ensuring appropriate intervention and

    documentation.

    • Conduct reviews of all member disenrollments to identify potential interventions and

    understand disenrollment trends.

    • Ensure compliance with standards set by LHH, the Care Management Agency (CMA),

    and the Department of Health (DOH).

    • Review and approve employee timecards, with oversight from Human Resources.

    • Support care managers in referring members to HCBS/CORE services and assist in

    maintaining monthly performance metrics.

    • Participate in committees or workgroups as requested by the Clinical Director.

    • Provide regular individual and group supervision, both scheduled and informal.


    Education and Experience Requirements:

    • Bachelor’s degree with 3+ years of relevant experience, OR

    • Licensed Registered Nurse (RN) with 2+ years of relevant experience, OR

    • Master’s degree in social work, human services, or public administration with 1+ year of

    relevant experience.

    • Minimum of 1 year of supervisory experience in care coordination or case management.

    • Completion of Care Coordination CORE Training within the first six months of

    employment.

    • Completion of CMA and LHH required trainings within the first two years of employment.
