The Community Health Worker Supervisor leads a team of Community Health Workers in addressing social needs of patients throughout MyMichigan Health’s service areas. Under supervision of the Manager Population Health, this position will oversee and support the CHW team and its focus of improving health outcomes, promoting wellness, and enhancing access to services. This role will execute CHW tasks including completing outreach to connect patients to needed health/social services. This person will be a key member of ambulatory patient-centered health teams and community collaborations and will work closely with the Community Health and Population Health teams.
Responsibilities:(60%)* Manage Community Health work queue referrals for assigned geographies. Conduct patient outreach to address identified social needs. Assist patients and families in addressing health disparities by connecting them with appropriate community resources to remove SDOH barriers. Establish/maintain positive relationships with community partners.
(40%)* Oversee the daily operations of assigned Community Health Workers. Develop and implement staff meetings, training, and planning structure designed to address health disparities identified through social drivers of health (SDOH) assessments. Support team members in the development of work plans and in navigating challenging or complex patient needs.
OTHER DUTIES AND RESPONSIBILITIES
Refer and link patients to preventive services through health screening and health care information
Promote wellness by providing locally and culturally appropriate information to patients
Advocate for patients when needed
Provide oversight, mentorship, and support to CHWs, ensuring program goals and patient needs are met
Maintain current knowledge of all SDOH recommendations and CMS requirements
Maintain current knowledge of CHW billing requirements and processes
Monitor staff activities to ensure compliance with policies, CMS requirements, guidelines, and quality programs/metrics
Identify training needs, arrange continuing education, and support ongoing professional development
Responsible for hiring, disciplining and discharging employees according to MyMichigan Health policy with input from the Manager Population Health
Responsible for written job performance reviews for all employees with input from the Manager Population Health
Utilizes the payroll system, manages staff schedules; inputs staff time off requests and approves employee hours in preparation of the biweekly payroll process
Identify and monitor KPI associated with CHWs; establish process improvement activities if off track
Assists in the annual budget process and planning
Matrix reporting of other CHW roles in health system to ensure alignment of CHW workflows and billing processes
Identifies and communicates any potential business opportunities to expand/enhance services and market share

Certifications and Licensures:Certification: Community Health Worker Core Competency certification is required within 6 months of hire or transfer.
Required Education:Education: High School Diploma or GED
Preferred: Bachelor’s degree in health, human, or social sciences
Other Information:EXPERIENCE, TRAINING AND SKILLS:
Experience in a health system, public health sector, or community benefit organization.
Preferred: Experience in managing a team and project development
Preferred: Three years of experience in health or social service delivery
Ability to manage and organize multiple projects with attention to detail
Ability to problem-solve independently and proactively
Considerable knowledge of service area and community support resources
Capability to help CHWs navigate complex social/health issues
Ability to explain health information to the public in a clear, concise, accessible manner
Strong interpersonal skills, can relate and communicate positively to staff and the general public
Knowledge of Microsoft suite, databases, and is able to learn other software as needed
Knowledge of trauma informed supervision

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
Overall vision and hearing is necessary with or without assisted device(s).
Frequently required to sit/stand/walk for long periods of time.
May require frequent postural changes such as stooping, kneeling or crouching.
Some exposure to blood borne pathogens and other potentially infectious material.
Must follow MyMichigan Health bloodborne pathogen and TB testing as required.
Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
