Position Summary:
We have an exciting opportunity to join our team as a Supervisor-Hospice -Full Time - Day Shift -Mon-Fri- 8.30am-4.30pm - Hospice - Full-Time - Day Shift.

In this role, the successful candidate Works collaboratively with the Manager/Assistant Nurse Manager of Hospice Services and is responsible and accountable for establishing and maintaining standards, related to supervision of referrals for Hospice services. Serves as a patient care advocate and role model in the performance of direct and indirect patient care and assumes leadership role and function in absence of the Manager.

Job Responsibilities:

• Performsotherduties asassigned

• Assists Manager in establishing, monitoring and evaluating standards of patient care focusing on measurable outcomes.Consistently addresses patient careneedsinatimelymanner.Screen inquiries and referrals and processes community requests for Hospice service inaccordance with Agency policies and procedures. Completes a comprehensive assessment of patients referred to Hospice services from Hospitals, skilled nursing facilities, rehab facilities and from the community. Responsible for the planning to admit the patient to the Agency. Determines and ensures client eligibility and authorization to receive requested services and notifies all pertinent staff of approval or denial of requests.

• Participates and/or supervises multidisciplinary process improvement activities for program specific and hospital wide initiatives.Assures compliance with Joint Commission, Department of Health, NYS Department of Social Services and other regulatory agencies,as well as agency policies and procedures.Assists with attainment of specific goals and objectives related to referral management inaccordance with organizationaland agency strategic goals.Interprets, implements and promotes agency policy; recommends policy modifications.

• On-Call Duties: Collaborates on assessment of patient's clinical status with team members and attempts satisfactory resolution ofpatient clinical issuesinatimelymanner.Addresses patient teammember issuesfollowing established communicationhierarchy.Participates inidentification,investigation andresolution ofactual andor potential risk managementquality care ethical issues. Holds team members accountable.

• Assumesleadership roleandfunctions intheabsence ofManager.Ensures theprovision ofageappropriate and safe care service (population served). Maintain clinical knowledge base appropriate topatient population.

• Responds inatimely manner toorganizationalinformation requests forreports, anddata.Consistently utilizes computer to communicate, document, track and evaluate information.

Minimum Qualifications:
To qualify you must have a Bachelor's Degree Nursing. Current and valid NYS Registered Nurse License. 3-5 years in Clinical Nursing. 1-2 years in Community Health nursing. Excellent communication (both oral and written), interpersonal, organization, leadership skills. Basic computer skills..

Required Licenses: Registered Nurse License-NYS

Preferred Qualifications:
Master's Degree Nursing,Health Education, Community Health, or Public Administration. 9-11 years Nursing (at least one year in Homecare,Hospice or LTHHCP setting).

The salary range for the role is $84,956.87 - $143,709.52 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
