The primary duty of the Support Coordinator is to coordinate services for individuals with developmental disabilities and their families within the Family and Community Living Support program. This includes gaining access to the care and services they require through an assessment of need, development of plan of care, coordination of services and ongoing monitoring of needs and services. The Support Coordinator must ensure that services are provided in a way that best meets individual needs, in a manner that maintains compliance with applicable rules and regulations of certifying and oversight agencies, that maximizes available resources and that ensures the health, safety and welfare of the persons served.
This position would involve working with children and adolescents that live in the community. These positions require a human service-related, four-year bachelor’s degree and at least one year of experience working with persons with disabilities. Applicants must be at least 21 years old. A strong preference will be given toward applicants with coursework which will lead to certification as a federally defined Qualified Developmental Disabilities Professional (QDDP).
