Job Title: Supportive Care Nurse Status (Full-time, Part-time, PRN): Full-time

Job Overview:

In this role, you will oversee PCA’s, Med Tech’s, and CNA’s ensure high quality resident care, and maintain compliance with all Homecare regulations. The Supportive Care Nurse will coordinate and supervise daily clinical operations, provide direct resident care as needed, and support a positive work environment for staff and residents.

Responsibilities:

Resident Assessments & Plans of Care

  • Conduct comprehensive initial nursing assessments for all home care clients.
  • Develop individualized Plans of Care in compliance with NC DHSR standards.
  • Review and update Plans of Care at least every 90 days or when a change in condition occurs.

Supervision of Aides

  • Provide initial and ongoing supervision of Personal Care Aides, Med Techs, and PCAs.
  • Conduct supervisory visits at least every 90 days for personal care services and every 60 days for nursing tasks delegated to unlicensed staff.
  • Document supervisory visits and maintain compliance records.

Delegation & Oversight

  • Delegate nursing tasks appropriately to unlicensed personnel in accordance with NCBON delegation rules.
  • Ensure aides and med techs perform care in line with Bermuda Village policies, training, and state requirements.
  • Provide guidance, mentoring, and support to the home care team.

Compliance & Quality Assurance

  • Ensure services are delivered in accordance with 10A NCAC 13J, HIPAA, and Bermuda Village policies.
  • Participate in quality assurance/performance improvement (QAPI) programs.
  • Assist leadership in preparing for audits, surveys, and regulatory reviews.

Communication & Coordination

  • Serve as liaison between physicians, residents, families, and the Bermuda Village care team.
  • Provide education to residents and families regarding the scope of home care services.
  • Communicate effectively with staff and leadership regarding care delivery and resident outcomes.
  • Supervise teams of PCA’s and Med Techs to ensure patient care needs are being met.
  • Keep proper records of care as required by the facility. Complete all forms and documentation as required.
  • Assessing and monitoring wounds; cleaning and bandaging wounds.
  • Working collaboratively with other members of the patient care team to determine if additional treatments or changes in care are required.
  • Immediately report any observations of concern/changes in residents’ condition to the Health Care Administrator, or On-call providers, per physician group (some residents may have different providers).
  • Vital sign monitoring will be performed as needed or directed by regulations, policy or primary care provider’s orders.
  • Be able to apply the community’s accident, disaster, fire safety, elopement and emergency procedures for protection of residents.
  • Be capable of performing CPR upon successful completion of CPR Certification.
  • Be knowledgeable of the residents’ background, preferences, habits and interests and preserve confidential information about each resident.
  • Understand and follow safety and infection control policies and procedures and readily report.
  • Respond to emergency call signals promptly.
  • Report incidents to the Health Care Administrator promptly and complete appropriate paperwork as required in a professional manner.
  • Readily and directly report any verbal or physical abuse of a resident, inappropriate use of resident or facility property or lack of or inappropriate care of a resident to the Health Care Administrator.
  • Help to properly maintain all equipment and ensure work areas are clean and safe.
  • Assist in cleaning residents’ rooms, dining areas, and public areas as indicated by regulations.
  • Ability to communicate effectively with residents, families, staff, vendors and the general public.
  • Must be willing to be on-call in rotation with our Administrative Nursing Team.
  • Must have compassion for and desire to work with the elderly and understand that for each resident the facility is considered the Resident’s home.
  • Must demonstrate the ability to work responsibly as a team member as well as an individual.
  • Must be honest, ethical, fair, dependable, respect confidentiality and the rights and privacy of others.
  • Responsible for ensuring that all employees are providing excellent customer service to internal and external customers.
  • Perform job duties for residents and with team members in a courteous and professional manner.
  • Take the initiative to ensure resident safety and satisfaction is a priority.
  • Meet and greet visitors in a friendly, helpful manner (visitors include anyone who visits the community; specifically, current residents’ families and friends, prospective residents and their families, referral sources, vendors and regulators).
  • Answer phones appropriately, according to company and community standards.
  • Communicate professionally and cooperate with supervisors and all community personnel.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and, at times, hostile people within the community.
  • Ensure compliance and understanding of all regulations regarding residents’ rights.
  • Perform other duties assigned.

Required Qualifications:

  • Current Registered Nurse (RN) license in good standing with the North Carolina Board of Nursing.
  • Minimum 1 year of nursing experience (home care, long-term care, or community health preferred).
  • Knowledge of NC DHSR home care regulations (10A NCAC 13J) strongly preferred.
  • Demonstrated ability to supervise, delegate, and support unlicensed personnel.
  • Strong organizational, leadership, and communication skills.
  • Must meet all health and safety requirements.
  • Must pass a criminal background check.
  • Must be able to perform duties and responsibilities, with or without reasonable accommodation.

Preferred Qualifications:

  • Pervious supervisor experience is preferred.

Physical Requirements & Work Conditions:

  • Must be willing to be on-call in rotation with our Administrative Leadership Team.
  • Schedule hours; possible overtime. May work in other positions temporarily when necessary.
  • Ability to conduct supervisory visits throughout the community campus.
  • Occasional need to lift or move objects up to 25 lbs.
  • Flexible schedule to meet regulatory and resident needs.
  • Is subject to callback during emergency conditions (e.g., severe weather, evacuation, post disaster, etc.).
  • Is subject to injury from falls, burns from equipment, odors, etc., through the workday, as well as to reactions from dust, disinfectants, and other air contaminants.
  • May be subject to the handling of and exposure to hazardous chemicals.
  • Including but not limited to sitting/standing in place for short/long periods at a time, repetitive use of hands, fingers, wrists, arms, frequent walking, bending, reaching, stooping, squatting, pushing/pulling/lifting 25+lbs. High people interaction, bright and low lighting, loud noises, fluctuating temperatures, exposure to odors communicable illness/disease and other related infectious contagions found in a healthcare/residential setting.
