Job Overview – Supportive Housing Case Manager
Compensation: $26/hour
Location: Brooklyn, NY
Schedule: Monday to Friday (In-Office)

TAG MedStaffing is hiring a Supportive Housing Case Manager in Brooklyn, NY for our client, supporting case management and community-based services for individuals within a behavioral health environment. This role will work directly with residents to support wellness planning, housing stability, community integration, and access to social services and community resources. The ideal candidate is compassionate, organized, and committed to supporting individuals facing mental health, housing, and reentry-related challenges.

Responsibilities as the Supportive Housing Case Manager:
  • Case Management: Provide case management services and support residents in developing individualized rehabilitation, wellness, and housing support goals.
  • Community Resource Coordination: Connect residents with medical, mental health, substance use, vocational, food, and other community-based support services.
  • Resident Engagement: Build supportive relationships with residents while promoting independence, recovery, self-determination, and community integration.
  • Group Facilitation: Conduct wellness management and support group meetings focused on life skills, recovery, and community living.
  • Housing Support: Conduct apartment inspections, monitor living conditions, and support residents in maintaining stable and safe housing environments.
  • Documentation & Reporting: Complete service plans, progress notes, housing support documentation, and related case management reporting activities.

Qualifications for the Supportive Housing Case Manager:
  • Education: Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or a related field is required.
  • Experience: 1+ years of experience working with individuals experiencing mental health challenges, substance use issues, homelessness, or reentry-related needs is preferred.
  • Industry Knowledge: Experience within supportive housing, case management, community outreach, or social services environments is strongly preferred.
  • Technical Skills: Proficiency with Microsoft Office Suite including Word, Excel, Outlook, and basic case management documentation systems is required.
  • Skills & Attributes: Strong communication skills, empathy, organizational abilities, crisis management skills, professionalism, and the ability to build rapport with diverse populations are required.
  • Bilingual Skills: Bilingual English and Spanish language skills are preferred.
