Supportive Services Coordinator - Home Delivered Meals

Job Summary

Coordinates and oversees entire Home Delivered Meal program for Prime Time. This position requires adherence to the Rules and Regulations through the State of Ohio for the Area Agency on Aging, Region 9 (AAA9).

Required Education and Experience

• High school diploma or equivalent, Associates Degree preferred
• 
  
• Valid Ohio Driver's License
• 
  
• Must be able to lift 20 lbs
• 
  
• Must be able to pass a background check, drug screen, and alcohol test
• 
  
• Must be a quick learner
• 
  
• Must be able to use a computer and learn software programs
• 
  

Training, Required Licensure and Certifications

• A neat appearance, friendly manner, and patience working with senior citizens
• 
  
• Ability to communicate effectively over the phone with clients and with co-workers

• Ability to communicate effectively with AAA9 staff

Essential Key Job Responsibilities

• 
  
• Works with new and existing HDM clients and PASSPORT clients
• 
  
• Compiles and enters data into monthly, quarterly, and annual reports as required
• 
  
• Talks to clients and adds, removes, or changes client information in database
• 
  
• Updates and maintains waitlists when appropriate
• 
  
• Writes meal delivery update sheets and provides information to HDM Drivers daily
• 
  
• Enters data monthly into billing software for Area Agency on Aging, Region 9 (AAA9)
• 
  
• Maintains client files and required paperwork for AAA9 Audits and PASSPORT Audits
• 
  
• Adds, removes, or edits HDM and PASSPORT client information as necessary
• 
  
• Ensures HDM drivers maintain meal delivery sheets as required by AAA9
• 
  
• Updates and maintains Prime Time and AAA9 databases
• 
  
• Completes all requirements as a service provider through AAA9 for Home Delivered Meal and PASSPORT services
• 
  
• Works with HDM team to disseminate appropriate information relative to AAA9 requirements
• 
  
• Prepares, disseminates, and compiles HDM Surveys annually as required by AAA9.
• 
  
• Oversees information required for all audits performed by AAA9 / PASSPORT
• 
  
• Collects and maintains temperature sheets, HDM van checklists, and any information required for AAA9 audits
• 
  
• Prepares and submits monthly HEAP reports to AAA9 for reimbursement
• 
  
• Ensures HDM staff completes required education courses and Performance Self-Evaluations
• 
  
• Other duties as assigned
