About the Role
Community Living Services (CLS) is seeking a Support Coordinator to advocate for and coordinate services for children and adults with developmental disabilities. This role focuses on empowering individuals to live meaningful, independent lives through Person-Centered Planning, community inclusion, and strong collaboration with families, providers, and community partners.
What You’ll Do
Develop, implement, and monitor Person-Centered Plans
Coordinate and oversee services, supports, and entitlements
Advocate for individuals’ rights, preferences, and goals
Support community inclusion, relationship building, and self-advocacy
Assist with housing, transportation, employment, education, and income opportunities
Facilitate crisis planning, prevention, and resolution
Support health, safety, and emergency planning
Collaborate with families, providers, CMH partners, schools, and community resources
Maintain accurate documentation and compliance with Medicaid, HIPAA, and agency standards
Participate in required training and ongoing professional development
What We’re Looking For
Required Qualifications:
Bachelor’s degree in a human services field
Minimum of one (1) year experience supporting individuals with developmental disabilities
Meets Michigan Qualified Intellectual Disabilities Professional (QIDP) requirements
Strong communication, organizational, and problem-solving skills
Proficiency in Microsoft Office (Word, Excel, Outlook, Internet)
Preferred Qualifications:
RSST or applicable social work license
Experience with Person-Centered Planning and Self-Determination
Additional Requirements
Valid Michigan driver’s license with acceptable driving record
Reliable transportation with current registration and insurance
Extensive scheduled and non-scheduled travel within the service area
Flexibility in work hours to meet the needs of individuals served
