About the Role
Community Living Services (CLS) is seeking a Support Coordinator to advocate for and coordinate services for children and adults with developmental disabilities. This role focuses on empowering individuals to live meaningful, independent lives through Person-Centered Planning, community inclusion, and strong collaboration with families, providers, and community partners.
What You’ll Do
- Develop, implement, and monitor Person-Centered Plans
- Coordinate and oversee services, supports, and entitlements
- Advocate for individuals’ rights, preferences, and goals
- Support community inclusion, relationship building, and self-advocacy
- Assist with housing, transportation, employment, education, and income opportunities
- Facilitate crisis planning, prevention, and resolution
- Support health, safety, and emergency planning
- Collaborate with families, providers, CMH partners, schools, and community resources
- Maintain accurate documentation and compliance with Medicaid, HIPAA, and agency standards
- Participate in required training and ongoing professional development
What We’re Looking For
Required Qualifications:
- Bachelor’s degree in a human services field
- Minimum of one (1) year experience supporting individuals with developmental disabilities
- Meets Michigan Qualified Intellectual Disabilities Professional (QIDP) requirements
- Strong communication, organizational, and problem-solving skills
- Proficiency in Microsoft Office (Word, Excel, Outlook, Internet)
Preferred Qualifications:
- RSST or applicable social work license
- Experience with Person-Centered Planning and Self-Determination
Additional Requirements
- Valid Michigan driver’s license with acceptable driving record
- Reliable transportation with current registration and insurance
- Extensive scheduled and non-scheduled travel within the service area
- Flexibility in work hours to meet the needs of individuals served
