Additional dates can be added once a provider is credentialed

Setting: Hospital: Inpatient and same day Outpatient Requirements:

Active, unrestricted PA license
Truly Board Eligible or Certified Orthopedic Surgeon
Clean DEA, Malpractice & NPDB (no current suits, unresolved cases, or large payouts in previous 7 years)
Accredited Orthopedic Surgery Residency & Accredited Orthopedic Trauma Fellowship
