    Overview
    The Targeted Case Manager is responsible for assessing, coordinating, monitoring and evaluating services to clients with a mental health diagnosis and severe emotional disability. The TCM works closely with clients, families and service providers to ensure clients receive appropriate support to meet their needs. Caseload maximum of 25 clients. Full-time or part-time, flexible scheduling.


    Responsibilities

    1. Client Assessment and Intake:

    a. Conducts comprehensive assessments to determine client’s needs, strengths and resources.

    b. Engages clients and families to establish a trusting and supportive relationship.

    c. Develops individualized service plans based on assessment findings.

    2. Care Coordination:

    a. Identifies and coordinates appropriate services and resources based on client needs.

    b. Facilitates communication between clients, families and service providers.

    c. Ensures services are tailored to meet the needs of each client.

    3. Advocacy:

    a. Advocates for clients’ needs and rights within various systems.

    b. Assists clients in overcoming barriers to accessing services

    4. Monitoring and Evaluation:

    a. Regularly reviews and updates service plans based on clients’ progress and changing needs.

    b. Monitors client’s goals and adjusts interventions as necessary.

    c. Maintains accurate and timely documentation of all case management activities.

    5. Professional Development:

    a. Participates in ongoing professional development activities as needed.

    b. Attends regular supervision, staff meetings and training sessions as required.

    c. Maintains compliance with state Medicaid requirements.


    Qualifications:

    Education: Bachelor’s degree in social work, psychology, or human services field.

    Experience: One year of post graduate experience in working with children (or) Master’s degree required.

    Must successfully complete Targeted Case Management Training within first 6 months of employment
