Salary: $35,000 - $39,375 annually
Educational Requirements- Bachelor’s Degree in Human Services or a related field required
- Minimum of one year of post-graduate experience required
Duties & Responsibilities- Provide case management services to assigned consumers in accordance with individualized treatment and service plans
- Coordinate and monitor services to support consumers’ behavioral health, medical, social, and community resource needs
- Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements
- Collaborate with consumers, families, providers, and community partners to ensure continuity of care
- Assist consumers with accessing community resources, benefits, and supportive services
- Participate in treatment team meetings and contribute to care planning efforts
- Adhere to all Pathways policies, procedures, and confidentiality standards
- Perform additional duties as assigned within the scope of the position
Skills & Knowledge- Experience working in a behavioral health setting preferred
- Strong communication, organization, and interpersonal skills
- Ability to manage multiple tasks and maintain accurate documentation
- Competency in using computers, electronic health records, and other electronic devices required for job duties
- Ability to work independently while collaborating effectively within a multidisciplinary team
