Overview: We are seeking a dedicated Targeted Case Manager to join our team. This role plays a vital part in improving the quality of care for our clients by coordinating services, advocating on their behalf, and promoting satisfaction and cost-effective solutions. The ideal candidate will be responsible for managing the full cycle of client care from assessment through evaluation.
Key Responsibilities:
Conduct comprehensive social assessments for all enrolled clients.
Develop individualized service plans based on assessment findings.
Coordinate and monitor the delivery of services to clients.
Link clients to appropriate community resources and advocate on their behalf.
Assist clients with applications for benefits and support services such as Medicaid, Medicare, SNAP, SSDI, and VA benefits.
Maintain compliance with Medicaid guidelines and agency standards.
Meet agency productivity goals by delivering a required number of service units.
Submit accurate and timely documentation, reports, and case notes.
Uphold agency policies, procedures, and ethical standards in all client interactions.
Collaborate with supervisors, co-workers, and community partners to support client needs.
Qualifications:
Bachelor’s degree from an accredited institution in Social Work, Counseling, Psychology, Criminal Justice, Health Education, or a related human services field.
Minimum of two (2) years of experience working with behavioral health populations.
Completion of AHCA-approved Case Management Training within three (3) months of hire (or willingness to complete).
Strong written and verbal communication skills.
Ability to work independently and as part of a team.
