Summary/Objective The Case Manager is responsible for ensuring that the changing needs of the eligible individuals are addressed on an ongoing basis and that the individuals are provided with options that are appropriate and provided from a wide array of options within the individuals' surrounding communities, as well as provide support and supervision to targeted case managers within the organization. The Case Manager are expected to provide the individuals served with linkage, consultation, and referrals to service providers to meet their ongoing needs. This position requires close coordination and communication with managers and clinicians. Case Manager will be provided monthly supervision and assisting with on job training. In addition, this individual is required to work successfully with representatives from various communities, organizations and neighborhoods with respect to the diverse populations and cultures, serves and seeks to serve. The Case Manager will also adhere to the code of ethics of their profession.
